Pennsylvania is an Interstate Medical Licensure Compact member state and fully implemented the Nurse Licensure Compact on July 7, 2025, which gives nurses a multistate privilege into the state. Commercial coverage arrived with Act 42 of 2024, enacted from SB 739 and approved July 3, 2024: 40 Pa.C.S. section 4803 requires policies to cover medically necessary telemedicine and forbids excluding a service solely because it was delivered remotely, while payment stays in accordance with the terms negotiated between insurer and provider - coverage parity without a rate mandate.
Prescribing here is stricter than in the compact states around it. State Board of Medicine regulations at 49 Pa. Code section 16.92(a)-(b), amended December 21, 2024, withhold a controlled-substance prescription unless the practitioner takes an initial medical history and performs a physical examination that objectively evaluates the heart, lungs and blood pressure. Telehealth may stand in for that first visit only inside an opioid treatment program, and then a full in-person examination is due within 14 days; the federal DEA flexibilities do not displace that state exam requirement. Physicians who practice near the border can apply for an extraterritorial license instead of a full Pennsylvania one.
